I understand that the Yellow Jacket Scholarship may vary based on the total financial aid package awarded with scholarship and grant funds. I understand that the total award will not exceed the cost of attendance at West Virginia State University.

I understand that if I am a recipient of additional scholarships, grants (federal or state) and other financial aid other than loans that this scholarship is subject to a reduction to ensure I am not over awarded as determined by the Office of Student Financial Assistance.

I understand that the Yellow Jacket Scholarship is limited to a maximum of eight (8) undergraduate semesters excluding Summer of attendance at West Virginia State University. The Scholarship funds may not be used to pay for summer school expenses.

I understand that successful completion of a minimum of 30 undergraduate semester credit hours annually from West Virginia State University evaluated at the end of each Spring semester and maintenance of a 3.00 cumulative grade point average (GPA) is required to retain this scholarship.

I understand that if I have not meet the criteria to retain the Yellow Jacket Scholarship and the scholarship was cancelled, that under extenuating circumstance I can initiate an appeal for consideration to have the scholarship reinstated.

I understand that Satisfactory Academic Progress will be verified at the end of each Spring semester.

I understand that the Yellow Jacket Scholarship is only awarded for the Fall and Spring semesters and excludes the Summer sessions.

I understand that I must complete the Free Application for Federal student Aid each academic year to be considered for this scholarship. Visit www.studentaid.gov. WVSU school code: 003826.

I understand that I may be required to participate in student support services and programs to ensure success (e.g. honors program, academic, mentor and time management workshops, etc.).

I understand that the final awarding of this scholarship is contingent upon verification of my final high school grade point average for New Freshmen and the overall college cumulative grade point average for Transfer Students.

I authorize West Virginina State University to release information about me regarding this scholarship in accordance with the Family Educational Rights in Privacy Act (FERPA) guidelines while protecting the confidentiality of personal information.
