- If you receive a “needs permission” error, check the top right of your internet browser. Ensure the top or primary account is your WVSU email.
2. Complete all questions and submit to close the form. Your answers are auto sent electronically.
Electronic PDF’s/ E-Signatures
Please follow the steps below

- You must open the file from your File Explorer/Manager to edit, sign and save properly.
- You may also open directly in Adobe Acrobat.

- Save the file.
- Send an email with the signed and saved file as an attachment to the remaining parties whom need to review/approve/sign.
- Each person that signs must also download the form as instructed in Step 1.
