As members of the WVSU community we each have a responsibility to report behaviors that potentially constitute discrimination, harassment or misconduct. We encourage any person who personally experiences or becomes aware of an individual who experiences such behaviors to utilize this form to notify the Office of EEO/Title IX. 

An individual should submit this form when they believe themself or another individual has been subjected to an unlawful employment practice based on their race, color, religion, sex (Including gender identity, sexual orientation and pregnancy), national origin, age (40 or older), disability or genetic information.

Even when unsure if a specific behavior constitutes a violation of employment practice, you are encouraged to complete and submit the form.

Once the completed form is submitted, it will be sent to the Title IX Office for review and appropriate response.
