Registering Other Decvices (Game Consoles, etc.)

In order to use this form, you must first obtain the MAC address of the device you wish to activate. This page provides information on how to obtain the MAC Address for various devices. This form will not activate your device, it only submits a request that the device be manually activated. Please allow up to 48 hours for your device to be activiated.

You must:

• A valid A#
• Your WVSU email address
• Be assigned to a room for the current semester
• If you specify "Other" as the device type, denote what it is

You will receive 2 emails during this process:

• The first being confirmation that you submitted this form
• The second being confirmation that your device has been activated.
